Leo Car Service is a Non-emergency Medical Transportation (NEMT). We provide Medicaid transport to Medicaid beneficiaries to and from medical services.

What is Non-emergency Medicaid Transportation.

States are required under federal regulation to assure necessary transportation for Medicaid beneficiaries to and from medical services.

How to Schedule.

Ordering Transportation is Easy

Order Medicaid transportation for your patients by either calling a specific county or borough number, faxing requests to (315) 299-2786 or logging on to MAS’ online transportation management system.

All trips should be scheduled as far in advance as possible. As a reminder, when scheduling trips, please have this information on hand:
  • Medicaid Number, ex: AB12345C
  • Name
  • Birth date
  • Address
  • Contact number
  • Reason for the transportation
  • Date and time of appointment
  • Location you will be attending, including the full street address
  • Transportation vendor with whom you prefer to ride
  • Any other special instructions needed for the trip
MAS may request additional information in order to schedule trips according to NYS guidelines.
If you need to cancel a trip, please do so as far in advance as possible.
To start the approval process, please call MAS at the number specific to your county or borough

to begin, or to find out if you are eligible for non-emergency Medicaid transportation. MAS deals frequently with enrollee-approved family members, medical practitioners and social services caseworkers who also set up appointments.
